Payment: The registration fee for this event is $50.00 (Lunch will be provided). Please make cheques payable to “Provincial Health Services Authority” and mail them with your registration form to the above address.

Receipts: Receipts will be available to pick up at the registration desk. CONFIRMATION WILL BE SENT BY EMAIL. PLEASE INCLUDE YOUR EMAIL ADDRESS.

Refund: Full refunds will be granted prior to September 28th, 2012. No refunds will be granted after September 28th, 2012 (however you may send a colleague in your place).

Registration: As there are a limited number of seats in the venue, please register early. The Final Registration Date is September 28th, 2012.
